Please have the following ready to quickly and easily complete the Kahuna certification application:

  • Full legal name of the business and d.b.a
  • Federal Tax ID number or owner's SSN
  • Criminal background check information on at least 1 principal of the business
    • Name, title, physical address
    • Social Security Number, date of birth
    • Gender
    • Ethnicity (required for background check, but not for Gold Service Certification)
  • Professional or business licensing information
    • License number
    • Issuing agency name, address and phone number
    • Original licensing date and expiration date
  • Insurance agency
    • Name, address and phone number
    • Contact person
  • Bonding agency (if applicable)
    • Name, address and phone number
    • Contact person
  • Three customer references
    • Name, address, phone number and contact person
  • Credit card information or check information (routing and account numbers)
